Health Requirements

Physical Examination:

New students, including preprimary, should present a certification (DH form 3040, or other comparable form) of school-entry health examination by the first day of school. However, under hardship circumstances, a new student may be given a 30-day grace period when necessary to obtain a school physical. Documentation must be provided by 30 days, or the student will be temporarily excluded from school pending documentation of the exam. S.1003.22(1), F.S.

Emergency Health Information:

Emergency health information should be updated annually in your Blackbaud account, for each student, listing contact person(s), family physician, allergies, significant health history, and permission for emergency care. Additional documentation may be required for the clinic.

Immunizations:

According to the State of Florida Immunization Guidelines, “prior to entry, attendance, or transfer to preschools, schools (K-12), licensed childcare facilities, and family daycare homes, each child shall have on file a Florida Certification of Immunization, DH 680.”

Every student shall have an up-to-date Florida Certification of Immunizations on file. You can obtain a DH 680 or DH 681 from the Health Department or your pediatrician. Parents of new students and students with updated immunizations, required for kindergarten and seventh grades, should upload their immunization documents directly into their Blackbaud account.
